National review into the murders of Arthur Labinjo-Hughes and Star Hobson

The National Panel carried out a review into the circumstances leading to the sad deaths of Arthur Labinjo-Hughes and Star Hobson which sets out recommendations and findings for national government and local safeguarding partners to protect children at risk of serious harm.

Serious Case Reviews

A local safeguarding Children Board is responsible for initiating a Serious Case Review (SCR) in circumstances where there has been a death of a child or young person and abuse or neglect is known or suspected, or where there has been a serious injury and there are concerns about interagency working.

The purpose of a Serious Case Review is to:

  • Establish whether there are any lessons to be learnt from the case
  • from the way in which local professionals and organisations worked together to safeguard and promote the welfare of children and young people, identify clearly
    • what those lessons are,
    • how they will be acted on,
    • what is expected to change as a result and within what timescale,
  • and as a consequence, improve inter-agency working to better safeguard and promote the welfare of children and young people.

Sometimes we contribute to a Serious Case Review in another LSCB area and we will publish these on this website.  When a Serious Case Review is completed and lessons identified, the report is published on this page of our website for 12 months. For copies of reports older than this please contact us.

    Learning Lessons Reviews

    Reviews of cases that do not meet the criteria of a Serious Case Review, but that ‘can provide valuable lessons about how organisations are working together to safeguard and promote the welfare of children’ (Working Together 2018) are also carried out by the Sheffield Children Safeguarding Partnership (SCSP) (formerly Sheffield Safeguarding Children Board - SSCB).  In these cases a Learning Lessons Review or Case Review may be carried out to pull out key learning and develop recommendations which are monitored by the SCSP.
